Where to get bank transfer details after losing my phone

I just lost my phone while on holiday in Cambodia. Unfortunately I wont get the replacement SIM until I get home next year to UK.

How can I do bank transfer to my revolut card if I don’t have access to the revolut app anymore to find out the correct bank sort code/account number?

Any help would be most useful?
