chargebacks process

Hello, i was asked by your customer service to fill the chargeback forms. However everytime i try to fill the document it request to sign in first by entering the phone number and applying the code but each time i gote a code on the phone, i go back to the first page and received a securty warning? How can i fill this document if eachtime it does not let me pass the first page?
